This below form is used to collect the Educational Interpreter's observations of the D/deaf/hard of hearing student's use of interpreting services and communication strategies.

Directions for making this Google form your own working copy:

  1. Click on the three vertical dots in the upper right hand corner of your page and select Make a copy to add it to your personal Google Drive folder.

  2. Click on Responses at the top middle of your page then click on the three vertical dots to set your form to Accept Responses, select whether you wish to Get email notifications for new responses and/or to create a response spreadsheet by Selecting Response Destination.

  3. Click on the Questions tab at the top middle of your page to edit text to meet your needs.

  4. Share the URL to your form with colleagues by clicking on the purple Send button at the top right of your page, click on the Link icon, check the checkbox to Shorten URL, then Copy and Paste the URL into your email, documents, etc.
